Tissue Expander Breast Reconstruction

Tissue expander / implant reconstruction- uses a tissue expander to stretch the remaining skin and the overlying pectoralis muscle, to allow the insertion of a silicone breast implant at a second stage. This procedure is best in women with small to moderate sized breasts, those undergoing bilateral mastectomies, and when there is no radiation planned.

One Stage Implant Breast Reconstruction

One-stage implant reconstruction with Alloderm®- often combined with nipple-sparing mastectomies and in prophylactic cases. These patients don't need a second stage operation unless minor revisions are necessary.

Nipple Sparing Mastectomy Reconstruction

Implant reconstruction after nipple-sparing mastectomy - this can often be performed as a one-stage procedure.

BRCA Mastectomy Reconstruction

When women test positive for the BRCA gene, they have a high lifetime risk of breast cancer.  Many women often decide to undergo bilateral prophylactic nipple-sparing mastectomies to significantly reduce their risk of breast cancer.  Bilateral mastectomy reconstruction in these patients is usually performed with implants in a single-stage procedure.

TRAM and DIEP Flap Reconstruction

Flap reconstruction with skin and fat from the abdomen is called a TRAM flap.  When the blood supply is left attached and one of the abdominal muscles are used as the blood supply, it is called a pedicled TRAM flap.  When only a small portion of the muscle is taken, it is called a free TRAM flap.  This is a microsurgical procedure where the flap is completely detached from the abdomen and sutured into little blood vessels in the chest.  When no muscle is taken from the abdomen, and only the skin and fat are taken with their blood supply, this is called a DIEP flap.  The DIEP flap is also a microsurgical procedure and leaves the most strength on the abdomen since no muscle is taken with the flap.  Dr. Jandali almost exclusively performs DIEP flaps due to the benefits from preserving all abdominal muscle.

Latissimus Flap Reconstruction

Latissimus dorsi flap with a silicone breast implant- this operation is often used in revision cases or when a previous implant reconstruction has failed due to infection or radiation. This flap takes skin and tissue from the back and transfers it to reconstruct the breast.
